Qualifications

• Master Degree Nursing and managed care experience preferred.

Responsibilities

• Oversee daily operations of Case Management teams and Continuum of Care.
• Lead orientation and ongoing staff development for Case Management staff.
• Collaborate with hospital leadership to plan, direct, and evaluate care coordination and discharge planning.
