Job summary

Highmark Health is seeking an experienced Case Manager to provide comprehensive case management for members with complex health needs. You will assess health status, develop and implement care plans, coordinate services, monitor progress, and advocate for cost-effective, high-quality care outcomes.

You will collaborate with physicians, other providers, and the care team, document all activities in the EHR, and mentor junior staff.

Qualifications

• 5 years in clinical/case management or related health operations
• Experience serving diverse populations
• Advanced training in CBT, motivational interviewing, or DBT
• RN with CCM or related certifications preferred
• Knowledge of healthcare systems and case management principles

Responsibilities

• Manage caseloads of members with complex health needs
• Conduct comprehensive assessments and develop care plans
• Coordinate services and resources for access to care
• Monitor progress and adjust care plans as needed
• Advocate for members and communicate with physicians and providers
• Document activities in compliance with policies and HIPAA
• Mentor lower-level Case Managers
