FOR INTEGRATED CASE MANAGEMENT, AND RESIDENTIAL: Provides community-based mental health case management. Degree and scope of service intervention vary based on program as well as the changing needs of the individual.
FOR ADOLESCENT PARTIAL CARE, ADULT PARTIAL CARE, AND ACUTE PART HOSPITAL: Responsible for providing direct care services including individual and group counseling, support and other activities; participating in comprehensive assessment, development of treatment and discharge planning; maintaining clinical documentation, participating in and facilitating family and collateral meetings, team meetings and case conferences to ensure continuity of care.
