Job summary

Santa Clara Family Health Plan is seeking a Supervisor of Case Management to oversee daily caseload operations, develop new programs, and coordinate referrals across lines of business.

You will supervise Case Managers and Personal Care Coordinators, ensuring compliance with state/federal rules, SCFHP policies, and performance standards while collaborating with internal and external partners.

Qualifications

• Licensed clinician in California with unrestricted practice.
• Minimum two years in a supervisory/lead role in case management.
• Knowledge of Medicare/Medi-Cal regulations and NCQA standards.
• Medical management and care coordination experience preferred.
• CMSA Case Management Certification preferred (not required).
• Bi-lingual abilities in Spanish, Vietnamese, Chinese or Tagalog a plus.
• Proficiency in Outlook, Word, Excel and relevant CM software.
• Demonstrated accuracy and adherence to regulatory timelines.

Responsibilities

• Oversee daily case management operations for all lines of business.
• Supervise Case Managers and Personal Care Coordinators.
• Develop and implement new programs and workflows.
• Coordinate referrals and ensure regulatory compliance.
• Collaborate with internal departments and external partners.
• Support audits, corrective action plans and performance reviews.
