Job summary

Sanford Health, the nation’s largest rural health system, is seeking dedicated staff to join Good Samaritan West Union for FT variable shifts. You will work with an interdisciplinary team to support residents’ physical, psychosocial, and economic needs, and participate in care planning and conferences.

Requirements include an associate degree in Human Services, Social Work, or medical field, plus experience in long-term care or social services and state certification as applicable.

Qualifications

• Associate degree in Human Services, Social Work, or medical field required.
• Prior experience in long term care, social services or human services required.
• Must meet state certification requirements for Social Services.

Responsibilities

• Serves as a member of the interdisciplinary team in providing assistance with activities, social, emotional and economic concerns of the resident and family, thus enabling them to achieve or maintain an optimal level of functioning.
• Utilizes appropriate age-related resident care protocols relating to the physical and psychological needs of adult and geriatric patients/residents.
• Works closely with additional interdisciplinary team to achieve sound and timely outcomes.
• Formulates care plans, facilitates care conferences, investigates grievances and completes assessments.
