Job summary

Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings.

In this on-site role you will implement PT treatment plans, lead therapeutic exercise and gait training, perform functional activities, document in EMR, and educate patients and caregivers while collaborating with an interdisciplinary team.

Qualifications

• Licensed Certified Physical Therapist Assistant (CPTA) required or eligible.
• Experience with elderly populations in SNF/long-term care preferred.
• Strong documentation and communication skills.

Responsibilities

• Implement PT treatment plans to improve mobility and safety.
• Lead therapeutic exercise and gait training for seniors.
• Perform functional activities and patient education.
• Document accurately in the EMR and collaborate with the team.
