Responsibilities include developing PT plans, collaborating with care teams, and mentoring staff to uphold C.A.R.E. values across Indiana facilities.

Qualifications

• Current and valid Physical Therapist license in the state of Indiana.
• Ability to conduct thorough assessments and accurately document changes in resident conditions.
• Strong passion for geriatric nursing and commitment to senior care excellence.
• Demonstrates C.A.R.E. values to residents, family members, customers and staff.

Responsabilités

• Develop and apply physical therapy plans to promote residents' physical health.
• Collaborate with nursing and facility teams to coordinate resident care.
• Mentor and support staff as part of ASC Therapies and Wellness initiatives.
