Qualifications

• Graduation from an accredited physical therapy program.
• Active or pending state PT license.
• Interest in working with the senior population and helping them regain independence.
• Superior customer service and interpersonal communication skills.
• Strong communication and interpersonal skills to work with patients, families, and colleagues.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and functional impairments.
• Develop and implement personalized treatment plans to improve mobility and function.
• Deliver care at one or more assisted living locations.
• Provide therapeutic interventions such as manual therapy, therapeutic exercises, gait training, and balance programs.
• Promote safety and fall prevention by identifying risks and implementing measures.
• Provide care with independence while supported by Engage team for guidance.
• Educate patients and families on home exercise programs to maintain independence.
• Document patient care in EMR following state and facility standards.
• Engage in ongoing professional growth through education, in-service meetings, and workshops.
• Develop bonds with patients, families, doctors, and team members for collaborative care.
