Candidate Consult in Ocala, FL is seeking a Nurse Practitioner to provide comprehensive primary care for adults 65+. You will work in a value-based outpatient setting with a multidisciplinary team and autonomous practice.

Expect longer visits, focus on relationships, and guidance through evidence-based protocols. The role emphasizes patient advocacy, chronic disease management, and a supportive culture that values teamwork and clinical excellence.

Qualifications

• Licensed APRN in Florida and graduate of an accredited nursing program.
• 1–3 years of primary senior care experience preferred.
• Experience managing diabetes, COPD, CHF and hypertension.
• Proficient in EMR documentation with strong organizational skills.
• Spanish language skills preferred but not required.

Responsibilities

• Provide comprehensive primary care exclusively for seniors (65+).
• Longer, unrushed visits to foster meaningful patient relationships.
• Collaborate with physicians and multidisciplinary care teams.
• Apply evidence-based protocols to inform patient-centered decisions.
• Support care coordination within a value-based care model.
