Meals on Wheels Yolo County, Inc. is seeking a Community Health Worker to provide compassionate, person-centered support to seniors 60 and older across Yolo County. The CHW reports to the Programs Director and helps nourish health, independence, and safe aging in place.

The role is full-time, in-field with occasional office presence in Woodland, Winters, and other sites. Candidates should have a high school diploma, strong communication skills, and a valid CA driver's license.

Qualifications

• High school diploma or equivalent required; Associate’s Degree preferred; experience with seniors and outreach desirable.
• Ability to lift up to 50 lbs., stand for extended periods, bend below the waist, and reach overhead.
• Independent and adaptive work style with minimal supervision; flexible and patient in varied situations.
• Proficiency with or willingness to learn electronic systems; Microsoft Office; Google Workspace familiarity preferred.
• Valid California Class C driver’s license with satisfactory driving record.

Responsabilités

• Conduct participant intake and assessments to identify needs, strengths, barriers and goals.
• Manage an assigned caseload of seniors; conduct regular check-ins and coordinate ongoing services.
• Coordinate referrals and maintain an up-to-date directory of senior resources.
• Build relationships with seniors, caregivers, volunteers, colleagues and community partners.
• Maintain accurate case records and communicate needs to supervisors; attend staff meetings.
• Supervise Community Navigator(s) in shared responsibilities.
