Centene Corporation in New York, NY seeks an experienced RN for complex care management to develop personalized care/service plans and educate members and families on benefits and options. Requires NY RN licensure and a degree in nursing. Hours are Monday–Friday, 8am–5pm EST, with potential hybrid/work-from-home options.
The role focuses on coordinating care, collaborating with providers, and ensuring compliant, high-quality member services while supporting leadership on program improvements.
Position Purpose: Assesses, plans, and implements complex care management activities based on member activities to enable quality, cost-effective healthcare outcomes. Develops a personalized care plan / service plan for care members, addresses issues, and educates members and their families/care givers on services and benefit options available to receive appropriate high-quality care.
Key Details: This position requires NY RN State Licensure. Hours are Monday - Friday 8am to 5pm EST.
Education/Experience: Requires a Degree from an Accredited School or Nursing or a Bachelor's degree in Nursing and 4 – 6 years of related experience.
License/Certification:
Pay Range: $75,300.00 - $135,400.00 per year
