Job summary

Quality Moments Behavioral Health is seeking a compassionate Senior Care Case Manager to provide therapeutic activities and companionship to individuals in skilled nursing facilities. You will engage with seniors, support their social, emotional, and behavioral well-being, and collaborate with care teams.

Must have a Diploma or GED, valid driver's license, and experience with Alzheimer’s patients; prior LTC or CNA/STNA background preferred.

Qualifications

• Must be at least 21 years of age.
• Desire to improve quality of life for aging adults.
• High level of commitment and dependability.
• Strong interpersonal communication skills.
• Experience working with Alzheimer’s patients.
• Past nursing or long-term care experience preferred.
• Valid driver’s license and reliable transportation.
• Experience using EHR systems and professional documentation.
• Background in social work, counseling, or related field preferred.
