Job summary

Commonwealth of Pennsylvania's Monroe County Area Agency on Aging is hiring an Aging Care Manager 2 to help older adults navigate services, plan care, and connect with community resources. You will assess needs, coordinate supports, and document cases in SAMS and PIA while maintaining compassionate communication.

The role offers a standard 8:00 AM–4:30 PM schedule, possible after-hours as needed, travel requirements, and a chance to strengthen the community through direct guidance and thoughtful

Qualifications

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

Responsibilities

  • Consumer Assessment: Gather information about consumers and their situations to identify needs and locate appropriate services
  • Care Management: Provide support to consumers and families to help them adjust to personal or social challenges
  • Home Contact: Make home visits and phone calls in alignment with established plans and agency policies
  • System Documentation: Complete required assessments and maintain accurate records in SAMS and PIA systems
  • Benefit Assistance: Help consumers understand and receive benefits and provide information on relevant laws
  • Community Coordination: Work with community resources to ensure consumers can access available services
