Columbus House, Inc. in New Haven, CT seeks a Family Triage Coordinator to supervise residential staff and coordinate services for mothers and children at the Family Triage unit (40 capacity).

This seasonal, full-time position runs November 15, 2026 to April 15, 2027. Under the Manager of Case Management Services, you will provide supervision, training, and program management, ensuring safety, documentation, and collaboration with social service providers.

Qualifications

• Bachelor's degree in human services required.
• Minimum 2 years’ experience in human services or 5 years as a case manager.
• Strong organizational and time-management skills.
• Excellent written and oral communication in English.
• Proficiency with Microsoft Office and local social service networks.

Responsabilités

• Create strength-based person-centered approaches.
• Provide linkages to clients for social, mental health, education, vocational services.
• Coordinate with other agencies and social service providers.
• Enforce program rules and supervise resident chores and house meetings.
• Train and supervise staff; provide oversight and professional development.
• Attend agency and program meetings; maintain documentation and notes.
