Job summary

Greater Lawrence Family Health Center (GLFHC) is seeking a Community Health Worker/SDOH Data Entry Specialist to join our Community Support Services team in Lawrence, MA. The role focuses on conducting Social Determinants of Health screenings and ensuring accurate data entry in the EMR.

You will collaborate with care teams, maintain confidentiality, and support data quality, reporting, and program outcomes while upholding HIPAA standards.

Qualifications

• High school diploma or GED required.
• 1–2 years of data entry experience.
• Healthcare/public health/social services experience preferred.
• Strong attention to detail and accuracy.
• Proficiency in Microsoft Office (Word, Excel, Outlook).
• Experience with EMR/EHR or data-tracking platforms preferred.

Responsibilities

• Conduct SDOH screenings with patients in person, by phone, and via outreach activities.
• Engage patients in culturally responsive conversations to identify social needs and connect to resources.
• Enter and maintain SDOH screening results and demographic information in the EMR and tracking systems.
• Review documentation for completeness and accuracy, resolving missing or inconsistent information.
• Perform routine data quality audits to ensure compliance with standards.
• Assist with compiling reports on screenings, referrals, program outcomes, and productivity.
• Collaborate with CHWs, care coordinators, and clinical staff to ensure timely documentation.
• Maintain confidentiality and HIPAA compliance.
• Participate in Mobile Market registration and document SDOH results.
• Support quality improvement initiatives and other related duties.
