Job summary

Amergis is seeking a Speech-Language Pathologist for a school setting in Tempe, AZ. You will evaluate students and provide therapy services according to each plan of care and the physician's orders.

Qualified candidates must hold an active SLP licensure in the state of assignment and may need CCC-SLP depending on contract. CPR may be required; health certificates and regulatory screenings must be current. 18+ years of age, equal opportunity employer.

Qualifications

• Active Speech-Language Pathologist licensure in the state of assignment.
• CCC-SLP certification preferred and may be required by state/contract.
• Current CPR certification, if applicable.
• TB questionnaire, PPD or chest x-ray if applicable.
• Current health certificate as per contract or state regulation.
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Perform student evaluations and provide and document therapy services per plan of care and physician orders.
• Document therapy services in accordance with plan of care and orders.
• Collaborate with school staff and families to support student goals.
