Job summary

Town of Berlin seeks a School Nurse (CCT Academy School) to provide skilled nursing in school and home settings under the Director of Nursing. The role includes health screenings, first aid, and coordinating care with educators and medical staff.

The candidate must be a graduate of a National League for Nursing approved school, hold CT RN licensure, and have at least two years of RN experience within the last five years. Travel to homes may be required.

Qualifications

• Must be a graduate of a National League for Nursing approved school of nursing.
• Minimum of two current years of RN experience within five years prior to employment as a school nurse.
• RN licensure by the State of Connecticut required; must be able to communicate effectively with students, physicians and school staff.

Responsibilities

• Develops a plan of care with school personnel to maximize student educational benefit.
• Performs school nursing duties including first aid and health screenings (vision, hearing, height, weight, pulse, BP).
• Liaises between educational and medical personnel for special education placement and IEP development.
• Conducts physical screenings and home visits to monitor chronically ill students.
• Supervises Health Aids and volunteers; collaborates with therapists and social workers.
• Enters all pertinent information into the SNAP Software System.
