Job summary

Mentor Behavioral Health, part of Sevita, is seeking an Outpatient Therapist to provide therapeutic interventions to youth, families and adults in homes, communities, office, and virtually. You will develop treatment plans and coordinate with an interdisciplinary team.

Qualifications include a Bachelor's with LSW and at least 3 years’ experience with children/families, and a Master’s in human services with LPC or MFT. Sevita offers a full benefits package and ongoing professional development.

• Master’s degree in human services with licensure as LPC or MFT.

Responsibilities

• Conduct diagnostic evaluations.
• Provide therapy, family support, and crisis interventions to individuals, families and/or groups.
• Provide mental health consultation to stakeholders and treatment team members.
• Develop individualized treatment plans in coordination with an interdisciplinary team.
• Develop and coordinate discharge planning.
• Maintain timely and accurate health record documentation.
• Attend regular staff and clinical meetings and sessions with supervisor.
• Provide 25 service delivery hours per week.

Education

Bachelor's, LSW + at least 3 years’ experience with children /families

Master’s Degree in human services + LPC, MFT
