Job summary

The Hope Learning Academy Melrose Park is seeking an experienced Licensed Practical Nurse (LPN) to support the health needs of students on campus. The LPN will collaborate with the RN Case Manager to implement health care plans, monitor student well‑being, and ensure timely medication administration and documentation.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Illinois licensure (or license pending).
• Successful background clearance and drug/alcohol screening.
• Valid Illinois driver’s license and DCFS licensed driver status.

Responsibilities

• Assist the RN Case Manager in formulating and implementing client health care plans.
• Evaluate health care practices of the clients and encourage proper care techniques.
• Observe, identify and document health care needs and problems of clients.
• Pass medications as required, and prepare field trip medications timely.
• Perform nursing treatments, direct care, and other nursing tasks as required.
• Communicate and document orders, transfers, discharges, admissions and incidents.
• Is responsible for monthly physical assessments and weekly documentation of clients' status.
• Transcribe physician's orders promptly and accurately.
• Make rounds on units and classrooms regularly.
