Job summary

Meridian Health Services is seeking a full-time School Based Licensed Practical Nurse (LPN) for our School Clinics in Delaware County and Anderson Preparatory Academy. The role focuses on whole-person healthcare, integrating physical, mental and social well-being for students.

Schedule: Monday-Friday, 7:30-4:00; travel up to 10% for training and coverage at other locations as needed. The organization emphasizes teamwork with physicians and medical staff to deliver comprehensive health services

Qualifications

Must be a non-probationary Licensed Practical Nurse who has successfully completed an Accredited Nursing Program and is currently licensed by the State of Indiana.
Two (2) years nursing experience required.
Valid driver’s license, proof of auto insurance, reliable transportation, and a driving record meeting Meridian's driving policy.

Responsibilities

Responds to requests from students/caregivers/clients concerning medications and consults with medical staff.
Oversee documentation in MAR and provide corrections as necessary.
Document in school-based records and MHS electronic system when a patient visit occurs.
Phone in prescription orders to pharmacies as directed by medical staff.
Review lab and test results and report concerns to medical staff.
Obtain medical histories and perform physical assessments as directed; provide health screenings as needed.
