Job summary

Sedgwick County COMCARE is seeking a dedicated Case Management Specialist to deliver treatment planning and coordination for clients with mental health needs. The role includes case management, risk assessments, transportation of clients, and collaborative treatment planning.

Responsibilities include documenting progress notes per guidelines, maintaining timely records, and completing required orientations and screenings.

Qualifications

• Bachelor’s degree or equivalent work experience in human services.
• Must have valid US driver’s license without restrictions and current auto insurance.
• Must have personal vehicle for transporting materials/clients as needed.
• Completion of required orientations and background checks.

Responsibilities

• Provide service provision case management to remediate symptoms and support emotional/functional well‑being.
• Conduct risk assessments as needed to address client safety.
• Transport clients using a personal vehicle to attend medical appointments and related activities.
• Prepare and review Treatment Plans with client and family participation.
• Document progress notes and medical records per guidance and timelines.
• Enter AIMS data as needed.
