Job summary

Legacy Community Health in Houston is seeking a Nurse Practitioner or Physician Assistant for our School-Based Health Program to provide compassionate pediatric care in school clinics under physician supervision.

The role emphasizes patient education, care coordination, and collaboration with a multidisciplinary team; you’ll work Mon-Thu 7AM-4:15PM and Fri 7AM-3PM, with opportunities for mentorship and professional growth.

Qualifications

• Current Texas PA/NP license or eligibility and graduation from an accredited program.
• Experience with pediatric populations is preferred.
• Ability to work within school-based clinics and under physician supervision.

Responsibilities

• Administer routine pediatric care in school clinics.
• Follow Nurse Practice Act and clinic policies.
• Assess client needs and communicate improvement opportunities.
• Participate in clinic development with health team.
• Document care accurately and promptly.
• Consult and triage patients to ensure appropriate care.
• Evaluate lab values in collaboration with physicians.
• Educate patients on health maintenance and prevention.
• Attend interdisciplinary team meetings and improvement programs.
• Maintain professional and respectful conduct with patients and staff.

Schedule: Monday-Thursday (7AM-4:15PM) | Friday (7AM-3PM)

Key Responsibilities

• Administer routine pediatric care in school-based clinics, including medical procedures and drug treatments.
• Follow the Nurse Practice Act and maintain compliance with policies and protocols.
• Assist in assessing client services and report needs for improvement.
• Participate in clinic development and implementation alongside our health team.
• Document all client care accurately and promptly.
• Consult and triage clients effectively, ensuring they receive the care they need.
• Evaluate lab results and follow up on abnormal values in collaboration with the supervising physician.
• Engage in patient education about health maintenance and disease prevention.
• Participate in interdisciplinary team meetings and performance improvement programs.
• Demonstrate respect and professionalism toward all patients and staff, contributing to a positive work environment.

Minimum Qualifications

• PA: Current Texas state licensure or eligibility for licensure and graduation from an accredited program as Physician Assistant
• NP: Current license from the Texas Board of Nursing and completion of program at an accredited school.
• Provider shall obtain, maintain and participate in the Medicare and Medicaid Programs, workers’ compensation, other federal and state reimbursement programs, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, and other health benefit program requested by Legacy.
• PA: 1 year of medical experience in a physician’s office or equivalent combination of training and experience
• PA and NP: 2-5 years experience is preferred.
• PA and NP: School-based (SBH) clinic experience is highly desired.
• Must be able to use electronic medical record with proficiency.
• Ideal candidate is independent, enthusiastic and highly motivated.
• Must be able to manage multiple deadlines and prioritize.
• Bilingual in Spanish is a plus.
