Job summary

Heart of Ohio Family Health is seeking a Certified Nurse Practitioner to provide medical services in ambulatory and clinic settings in Whitehall, OH. Responsibilities

• Maintains strict patient confidentiality at all times.
• Completes patient history and physical.
• Documents all patient care provided according to policy and Standard of Care Agreement.
• Prescribes medications in accordance with Ohio codes and formulary.
