The role delivers evidence-based mental health services, develops and monitors behavior change plans, and provides education to team members. The position requires a Master’s degree in Social Work, LMSW licensure in Michigan, and a current BLS certification.

Qualifications

• Master’s degree in Social Work is required.
• State of Michigan – LMSW licensure required.
• BLS required at hire.

Responsibilities

• Serve as a member of the Interdisciplinary Treatment Team, diagnosing problems, formulating treatment plans, and evaluating progress.
• Assess clinical status and provide treatment recommendations for referred patients.
• Provide evidence-based individual and/or group therapy for patients and their families.
• Prepare case histories, psychosocial assessments and treatment plans; make referrals to mental health specialists as appropriate.
• Monitor and develop treatment plans to prevent deterioration or relapse.
• Maintain records and prepare verbal and written reports related to patient care.
• Engage in community outreach, collaborate with schools and community agencies, and participate in IEP meetings and related activities.
• Supervise interns as assigned.
• Participate in the evaluation of peers and support staff.
