Job summary

Simple Care Solutions is hiring board-certified Nurse Practitioners in the San Fernando Valley to conduct Medicare Advantage AWVs and HRAs in members' homes or through telehealth. Our office is in Van Nuys, and visits are kept local in the Valley.

You will screen for fall risk, cognitive status, depression, reconcile medications, and coordinate with primary care providers. Flexible schedule; paid per completed visit; bilingual Spanish preferred.

Qualifications

• Active California NP license and board certification.
• 6+ months of Annual Wellness Visit or wellness exam experience (preferred).
• 1+ year of Medicare Advantage in-home assessment experience (preferred).
• EHR experience (Epic, eClinicalWorks, Athena, or similar).

Responsibilities

• Perform Medicare Advantage AWVs and Health Risk Assessments in homes or via telehealth.
• Document HRA with accurate HCC diagnoses and complete medication reconciliation, fall risk, cognitive, and depression screening.
• Coordinate with the member's primary care provider on follow-up and care gaps.
• Choose visit type: in-home, telehealth, or both.
