You will join an established clinic with RN and medical assistant support, utilize EPIC EMR, and participate in a collaborative care-team model designed to serve a high-need community.

The position offers 36 patient-facing hours per week, a salary around $319,000 with potential quality/productivity bonuses, and access to behavioral health services
