Job summary

Heart of the Rockies Regional Medical Center's South Park Health Center in Fairplay, CO is seeking a full-time Physician Assistant or Nurse Practitioner to provide outpatient primary care to a rural community.

The successful candidate will practice independently with strong physician support, deliver care across the lifespan, and focus on preventive and chronic disease management in a well-established clinic.

Qualifications

• Minimum one year of clinical experience in a primary care setting.
• Licensed Family Nurse Practitioner (FNP) or Physician Assistant.
• 2+ years of primary care experience preferred.
• Comfortable practicing in a rural setting with broad scope.
• Ability to work independently with strong clinical judgment.

Responsibilities

• Evaluate, diagnose, and manage patients in outpatient primary care.
• Provide care to patients across the lifespan, from pediatrics to adults.
• Collaborate with physicians and clinic teams to coordinate care.
• Support preventive care, chronic disease management, and patient education.
