This role requires triage of phone calls and portal messages, education for patients with liver disease or transplant-related needs, medication management and prior authorizations, and collaboration with PCPs, specialists, infusion services, and transplant centers to prevent

Qualifications

• Knowledge of nursing process and practice.
• Uses a wide variety of technical equipment (stethoscope, thermometers, sphygmomanometers, electronic monitoring devices).

Responsibilities

• Triage and manage patient phone calls, portal messages, and clinical inquiries.
• Coordinate care plans, diagnostic testing, imaging, and laboratory monitoring.
• Educate and counsel patients with liver disease, cirrhosis, hepatitis, fatty liver disease, and transplant-related needs.
• Medication management, prior authorizations, and monitoring of treatment adherence and outcomes.
• Coordinate with primary care providers, specialists, infusion services, transplant centers, and ancillary departments.
• Review and follow-up of laboratory results and diagnostic studies.
• Ongoing patient management outside of clinic visits to ensure timely interventions and reduce avoidable hospitalizations.
