Job summary

Parkside Health and Rehabilitation Center is seeking an experienced Wound Care Nurse (RN) to provide specialized care and support effective wound healing for residents. This role emphasizes clinical accuracy, thorough documentation, and collaboration with the interdisciplinary team to achieve high-quality patient outcomes.

The Wound Care Nurse will assess wounds, develop individualized treatment plans, monitor healing progress, and ensure evidence-based dressings.

Qualifications

• BSN preferred; graduate of an accredited RN program required.
• Current, active RN license.
• Peripheral IV certification.
• Minimum of two (2) years of clinical nursing experience required.
• Experience with geriatrics or subacute populations preferred.
• Management or leadership experience a plus.
• Current CPR/BLS certification.

Responsibilities

• Maintain accurate and updated care plans for all skin impairments.
• Conduct weekly wound rounds with the wound consultant.
• Perform and document weekly wound measurements with accuracy and timeliness.
• Monitor and evaluate all new pressure-relating skin openings and determine root causes.
• Assess and manage surgical, arterial, venous, and other complex wounds.
• Ensure all treatments, changes, and follow-up needs are documented thoroughly.
• Educate residents, families, and caregivers on wound care procedures and expectations.
• Demonstrate wound care techniques to caregivers to support staff competency.
• Communicate with the interdisciplinary team to support coordinated care.

We are seeking an experienced Wound Care Nurse to provide specialized care and support effective wound healing for residents. This role focuses on clinical accuracy, consistent documentation, and collaboration with the interdisciplinary team to ensure high‑quality patient outcomes.

About the Role

The Wound Care Nurse is responsible for assessing wounds, developing individualized treatment plans, monitoring healing progress, identifying complications, and ensuring that all wounds are treated and dressed according to evidence‑based standards. This role supports residents through the full healing process with a focus on safety, precision, and consistent follow‑up.

Key Responsibilities
