Job summary

Responsibilities

• Rotate between clinic and IP rounds during an 8-hour shift.
• Provide wound care and hyperbaric care in a fast-paced clinic setting.
• Lead and coordinate care touchpoints for patients.
• Collaborate with multidisciplinary team to ensure direct patient care.
• Maintain a safe environment for patients and staff.
• Educate patients and families about continuum of care and discharge planning.
• Support effective communication and patient throughput.
