Job summary

Uptown Health Care Center in Colorado seeks a dedicated RN to plan and deliver nursing care to residents in accordance with applicable standards and regulations.

The role emphasizes assessment, care planning, interdisciplinary collaboration, and emergency response; requires Colorado RN licensure, CPR certification, and skilled nursing experience.

Qualifications

• 1-3 years' experience providing direct care to multiple patients/residents preferred; previous experience in Skilled Nursing environment desirable
• Nursing degree from accredited college or graduation from approved RN program required
• Current, unencumbered RN license to practice in Colorado
• Current valid CPR certification required
• Maintains knowledge of equipment setup and use (monitors, infusion devices, drains, etc.)

• Evaluates and monitors residents' condition and provides professional nursing care per care plan and doctor's orders
• Contributes to development and evaluation of care standards with the interdisciplinary team
• Initiates emergency support measures when needed
• Coordinates admission, transfer and discharge in compliance with regulations and policies
• Performs assessments to identify changes in physical or mental status of residents
• Ensures nursing history is documented in the medical record for all residents

RN POSITION SUMMARY

Plans and delivers nursing care to residents in accordance with current company, federal, state and local standards, guidelines and regulations to ensure that the highest degree of quality care and dignity is maintained at all times.

DUTIES AND RESPONSIBILITIES

• Demonstrates commitment to company's mission, values and standards of ethical behavior.
• Evaluates and monitors residents' condition and provides professional nursing care in accordance with care plan and doctor's orders.
• Provides input in the formulation and evaluation of standards of care in conjunction with facility interdisciplinary team members.
• Initiates emergency support measures when needed.
• Coordinates resident admission, transfer, and/or discharge including completing appropriate forms and required documentation in accordance with government regulations and company policies.
• Performs assessment functions including identification of changes in the resident's physical or psychological condition (i.e., changes in lab data, vital signs, mental status).
• Ensures that nursing history is present in medical record for all residents.

KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS
