Job summary

Haven Hospice in Gainesville seeks a compassionate Triage RN to provide after-hours guidance, scheduling, and support for patients and families. This PRN role focuses on coordinating care, answering calls, and ensuring timely admissions and on-call dispatches.

The position requires an active RN license with hospice experience. A compassionate communicator, you will collaborate with physicians and care teams to uphold Haven Hospice standards.

Qualifications

• Graduate of an accredited nursing school with active RN license.
• 1 year hospice nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Nursing Compact RN license required.
• Commitment to continuing education and clinical excellence.
• Knowledge of the hospice philosophy of care.

Responsibilities

• Answer hospice calls during nights, weekends, holidays, and emergencies.
• Schedule nurse visits, manage admissions, and dispatch on-call staff.
• Talk with patients and caregivers to assess needs over the phone.
• Communicate with doctors about patient updates and new orders.
• Give instructions per each patient’s care plan.
• Coordinate after-hours needs such as equipment, supplies, and meds.
• Work with facility staff during off-hours.
• Document all calls, care updates, and decisions clearly.
• Ensure all care follows state and federal rules.

Location & Schedule

Office Location: Gainesville

Schedule: PRN, Shifts available weeknight evenings, overnights and weekends. Must Be able to work a Minimum of 2 Shifts Per Biweekly Pay Period.

Responsibilities

• Answer hospice calls during nights, weekends, holidays, and emergencies (severe weather for example).
• Help schedule nurse visits, handle emergency patient admissions, and dispatch on-call staff.
• Talk with patients and caregivers to assess needs and provide support over the phone.
• Communicate with doctors about patient updates and get new care orders if needed.
• Give instructions and guidance based on each patient’s care plan.
• Coordinate after-hours needs such as equipment, supplies, and medications.
• Work with facility staff to support their patients during off-hours.
• Document all calls, care updates, and decisions clearly and accurately.
• Ensure all care and procedures follow state and federal rules.

Qualifications & Requirements

• Experience in hospice or similar setting preferred.
• Nursing Compact RN license required.
