Job summary

Four Seasons is seeking an RN to provide nursing coverage for hospice patients via phone and in-person visits, including admissions and after-hours triage. You will work within regulatory standards, coordinate with case managers, and collaborate with a multidisciplinary team to ensure quality patient care.

The role requires strong knowledge of HIPAA, Medicare participation, EMR systems, and excellent communication.

Responsibilities

• Provide nursing coverage to patients via phone and in-person visits, including hospice admissions
• Serve as first responder for after-hours symptom-based calls
• Guide patients and families and coordinate follow up with case manager
• Participate in hospice care implementation and coordination
• Assist with emergency admissions
• Assess patient physical and mental health and response to treatment
• Document nursing assessments and plans of care
• Initiate, deliver, and evaluate nursing actions
• Supervise and educate other staff providing nursing care
• Collaborate with health care providers on patient-centered plans

FT/PT/PRN: Full time

Shift: Friday, Saturday and Sunday 8AM to 8PM

Location: Serves Macon, Jackson, Clay, Swain, Graham, and Cherokee counties

Position Summary:

Providers nursing coverage to patients/families/community partners via phone & in person visits, including admission to hospice services, as needed.

• Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
• Serve as first responder for symptom-based calls from current patients after normal office hours
• Provide guidance to patients and families as needed or communicate need for case manager or designee follow up promptly
• Participate in implementation and coordination of patient’s hospice care
• Assist with emergency admissions
• Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
• Record and report the results of the nursing assessment.
• Plan, initiate, deliver, and evaluate appropriate nursing acts.
• Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
• Collaborate with other health care providers in determining the appropriate patient and family-centered plan.
• Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
• Provide teaching and counseling about the patient's health.
• Report and record the plan for care, nursing care given, and the patient's response to that care.
• Supervising, teaching, and evaluating those who perform or are preparing to perform nursing functions and administering nursing programs and nursing services.
• Providing for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.

Minimum Qualifications:

Experience:

• Two years of clinical experience preferred

Licensures & Certifications: (if applicable)

• Valid NC RN license
• Valid Driver’s License and proof of automobile insurance

Summary of Physical Requirements:

• Flexion/Extension of arms/wrist
• Hand dexterity to include gripping, pushing/pulling and writing
