Job summary

Niagara Hospice seeks a compassionate RN to join our Home Care team on a per-diem basis in Lockport, NY. The On-Call RN provides skilled hospice care, conducts assessments, coordinates services, and supports patients and families during on-call shifts.

Hours are Saturdays and/or Sundays 8:30am-4:30pm with a minimum of 2 shifts per month; additional weekend and overnight shifts may be available for pick-up to meet patient needs.

Qualifications

• Graduate of an accredited nursing program.
• Current, unrestricted NYS RN license and registration.
• Current NYS driver's license.
• Ability to travel throughout Niagara County service area.

Responsibilities

• Provide skilled hospice nursing services during scheduled on-call shifts, including assessment, direct and indirect patient care, symptom management, education, emotional support, and coordination of services.
• Respond to patient and family needs by telephone and in-person visits; collaborate with physicians, the IDT and clinical leadership to ensure timely hospice care.

Position Summary:

The Per Diem On-Call Registered Nurse (RN) provides skilled hospice nursing services to Niagara Hospice patients and their families during scheduled on-call shifts. The On-Call RN provides comprehensive clinical assessment, direct and indirect patient care, symptom management, education, emotional support, and coordination of services to promote patient comfort, safety, dignity, and quality of life.

The On-Call RN responds to patient and family needs by telephone and/or through in-person nursing visits and collaborates with physicians, the interdisciplinary team (IDT), clinical leadership, and other members of the care team to ensure timely and appropriate hospice care.

Qualifications:

• Graduate of an accredited nursing program.
• Current, unrestricted New York State Registered Professional Nurse (RN) license and registration.
• Current New York State driver's license.
• Ability to travel throughout Niagara County service area.
