Job summary

Affinity Hospice is seeking a registered nurse/case manager to provide hospice care in home and community settings, leveraging nursing expertise and family collaboration to meet patient needs.

This full-time role includes weekend on-call coverage and a comprehensive benefits package with PTO, health benefits, 401K, tuition reimbursement, and advancement opportunities.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current CPR certification.
• Two years of RN experience in clinical care, home health or hospice preferred.
• Must have a licensed driver with insured vehicle.
• Observation, verbal and written communication, problem solving and basic math skills.
• Able to lift/position/transfer patients.
• Able to lift supplies and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care with infection control measures (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Demonstrate commitment, professional growth and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities for patients/families.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management.

Position Summary:

The registered nurse/case manager plans, organizes, directs, and provides hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Hours / Schedule:

Full Time; Weekend On Call Position scheduled Friday 4:30pm - Monday 8:00am.

Minimum Qualifications:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse
• Current CPR certification
• Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Able to lift, position and/or transfer patients.
• Able to lift supplies and equipment.

Key Responsibilities:
