Job summary

Gentiva is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. The role involves direct patient care, triage, and coordination with the hospice team, families, and physicians.

The ideal candidate has an RN license in the state, 1+ year of nursing experience, CPR certification, and a compassionate, calm presence in urgent care. Valid driver’s license and EMR proficiency are required.

Responsibilities

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior while on call
• Provide direct patient care after normal business hours
• Serve as liaison between patients, families, physicians, and hospice team during shift
• Respond promptly to urgent calls, visits, and symptom management needs; guide care decisions
• Document clinical visits and communications accurately in the EMR
• Participate in quality assessment and performance improvement programs
• Work independently while coordinating with daytime care team

Overview

As an After Hours Hospice RN, you’ll respond to patient needs during evenings, nights, weekends, and holidays—bringing peace of mind and compassionate care when it’s needed most. Full-time weekend Baylor schedule with benefits and PTO.

What You'll Do as a Hospice RN – After Hours / On-Call

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior
• Provide direct patient care and support after normal business hours
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift
• Respond promptly and compassionately to urgent calls, patient visits, and symptom management needsEvaluate patient and family needs to guide clinical decisions and implement the plan of care
• Support and collaborate with an interdisciplinary group including LPNs/LVNs, CNAs, and other support staff
• Accurately document clinical visits and communications
• Participate in quality assessment and performance improvement programs
• Work independently while ensuring continuity of care and coordination with the daytime care team

About You

• Current RN license in the state of requested employment (or eligibility to obtain)
• 1+ years of nursing experience
• A hospice heart: compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and the ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes, facilities, or wherever they reside
• Valid driver’s license, automobile insurance, and reliable transportation
• Current CPR certification

Preferred Experience (Not Required)

• 1+ years of hospice, home health, oncology, or palliative care experience
• Familiarity with symptom management, triage, and after-hours home-based care
• Prior experience in on-call or overnight nursing roles
• Strong documentation and electronic medical records (EMR) skills
