Job summary

Matrix Providers is hiring a Registered Nurse (RN) Utilization Management in Fairfield, CA. The role is full-time with a Monday–Friday schedule and an hourly compensation of $67.38 plus $5.42 for health and welfare, paid bi-weekly, with a comprehensive benefits package including insurance and PTO.

Requirements include an Associate’s Degree in Nursing, ACEN/NLNAC/CCNE accreditation, active RN license, and 12 months of relevant experience in nursing/health care management.

Qualifications

• RN with an active license and nursing degree.
• Experience in case management for diverse patient populations.
• Knowledge of HIPAA and healthcare privacy requirements.
• Familiarity with CM components and care coordination.
• Ability to communicate complex medical information clearly.

Responsibilities

• Proactively identify and evaluate patients for case management.
• Develop patient-specific plans and coordinate care with providers.
• Coordinate referrals and ensure education and adherence to plans.
• Act as patient advocate and liaison to healthcare and community agencies.
• Attend rounds and planning meetings to support multidisciplinary care.
• Document assessments and track patient outcomes.
