Qualifications

• Graduate of an accredited School of Nursing.
• Associate’s degree in Nursing; BSN preferred.
• Successful completion of nurse residency, if applicable.
• Licensed Registered Nurse in CT; BLS certification.

Responsibilities

• Provides nursing care within a non-acute setting in collaboration with the inter-disciplinary team and provider according to the patient’s needs.
• Coordinates and provides holistic patient-centered care.
• Utilizes evidence-based practice to ensure safe quality care and customer service.
• Incorporates advocacy, science and ethics in daily nursing practice.
• Supports research utilization through exploring evidence-based practices.
• Evaluates and reviews the plan of care with the interdisciplinary team to meet patient and family needs.
• Identifies and documents problems and expected outcomes based on a plan of care.
