Job summary

ProMedica in Toledo is seeking an RN Acute Care Navigator to oversee patient care, coordinate progression, and ensure appropriate care levels for a caseload. You will optimize length of stay, prevent readmissions, and support value-based care programs, collaborating with interdisciplinary teams to develop discharge plans.

You will educate patients about medications and post-discharge care, monitor progress, and escalate issues to leadership as needed.

Qualifications

• Must have BSN or 5 years of nursing experience.
• Active, unrestricted RN license in state of practice.
• MSN or higher preferred.
• Experience in patient advocacy, care management, and knowledge of hospital/community resources.
• Case management experience.
• Certifications in care management (CCM, RN-BC, ACM or similar).
