Job summary

Responsibilities

  • Perform psychosocial and physiologic assessments and develop a care plan.
  • Collaborate with patients and families to set realistic outcomes.
  • Administer prescribed medications and IV solutions per policy.
  • Assess pain using standard scales and adjust care to promote comfort.
  • Prioritize care, considering cultural, ethical, and spiritual needs.
  • Report changes in patient condition promptly.

  • Performs psychosocial/physiologic assessments that integrate changing data.

  • Formulates a plan of care based on trends of similar patients, including patient education and discharge planning. Collaborates with patients/families to identify realistic desired outcomes.

  • Administers prescribed medications and intravenous solutions according to standards of care and hospital protocols.

  • Assesses pain level in all patients using \"0-10\", \"Faces\", or other approved scale to promote comfort and enable patients to participate in their healing. Provides timely/appropriate assessment/intervention. Identifies comfort function goals with patients.

  • Tailors and prioritizes caring practice to individual needs, including cultural/ethical/spiritual needs. Participates in the planning of routine transitional health acre needs. (i.e. treatment options, patient placement options, end of life options). Adapts standards and guidelines for individual patients.

  • Reports significant observed or reported alterations in the patient's condition in a timely manner.
