Qualifications

Responsibilities

• Support identification and resolution of gaps in care from hospitalization to ambulatory care.
• Lead development and implementation of complex care plans with interprofessional teams.
• Promote evidence-based practice and continuous learning; participate in journal clubs and practice changes.
• Analyze care standards gaps and provide feedback to improve patient safety and quality of care.
• Maintain safety: this is a safety-sensitive position and requires alertness.
