Qualifications

• Associate's degree or equivalent nursing experience required.
• Registered Nurse (RN) with progressive hospital/ambulatory experience.
• Strong customer service and communication skills.
• Ability to collect data, generate reports, and analyze via EMR.
• 2–3 years of clinical nursing experience in the specialty area.

Responsibilities

• Assess the patient's plan of care and manage resources to meet healthcare needs.
• Assess activation, literacy, and self-management; coordinate follow-up.
• Educate patient and family on diagnosis, treatment, and nutrition; promote self-management.
• Review care pathways and support data collection for value-based contracts.
