Qualifications

• Must have a current and valid license as a Registered Nurse issued by the State of California.
• BCLS upon hire and maintain current.
• Previous six months experience in Acute Care hospital experience or Long Term Care.

Responsibilities

• Provide direct care in the Skilled-subacute unit.
• Communicate changes in patient condition and test results with physicians and care teams.
• Follow medication safety practices and nursing policies.
• Maintain crash cart readiness and stock after procedures.
• Adhere to infection control guidelines and PPE usage.
• Give thorough shift reports to ensure continuity of care.
• Assist with tests and bedside procedures following time-out protocols.
• Ensure patient safety through proper body mechanics and safe environment.
• Document medications given and complete MAR/narcotic records.
• Recognize signs of abuse and report to Adult Protective Services as required.
• Plan discharge and patient/family education with proper documentation.
• Prioritize patient care based on needs and resources.
