Job summary

Signature HealthCARE of Cleveland, TN is seeking an experienced RN to join our skilled nursing and rehabilitation team. This role emphasizes direct patient care, medication administration, wound care, and adherence to care plans within a supportive, team-based environment.

You will supervise nursing assistants, manage documentation in the EHR, and collaborate with the interdisciplinary team to ensure high-quality resident outcomes.

Qualifications

• Current, active RN license in the state of practice.
• Experience in a supervisory or charge nurse role is preferred.
• Strong clinical observation, medication administration, and documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
