Qualifications

• Active state RN license.
• CPR/BLS Certification.
• Experience in Long Term Care or Skilled Nursing preferred.
• Excellent technical, assessment and documentation skills.
• Strong analytical and problem-solving skills.
• Ability to lift 25+ pounds.
• Ability to stand, bend, and walk for extended time periods.
• Team player and strong communicator.

Responsibilities

• Provide nursing care to meet resident needs per standards of practice, orders, and center policies.
• Plan nursing interventions and monitor implementation for each resident.
• Evaluate care effectiveness, identify problems and trends, and develop alternatives.
• Conduct family and resident education.
• Administer medications and treatments timely according to policy.
• Transcribe orders and chart progress notes accurately.
• Identify and report changes in condition to supervisor, physician and family.
• Monitor skin changes and follow skin management protocols.
• Coordinate rehabilitation with therapists to meet needs.
• Ensure CNA documentation and assignments are completed on time.
