Job summary

Jobtailor is seeking an RN with 3+ years of experience in case management and care coordination to manage member cases within licensure scope. The role involves supervising non-RN clinicians, developing and updating care plans, and performing direct clinical assessments with the care team.

The position emphasizes collaboration with Medical Directors and interdisciplinary teams to coordinate services and authorize referrals as needed, ensuring high-quality member care.

Qualifications

• Requires an RN and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role
• Current, unrestricted RN license in applicable state(s) required.
• May require state-specified certification based on state law and/or contract.
• MA/MS in Health/Nursing preferred.

Responsibilities

• Responsible for overall management of member's case within the scope of licensure.
• Provides supervision and direction to non-RN clinicians participating in the member's case.
• Develops, monitors, evaluates, and revises the member's care plan.
• Performs face-to-face clinical assessments for identification, evaluation, coordination and management of member's needs.
• Coordinates care in conjunction with the member and the health care team.
• Assists with implementation of member care plans by facilitating authorizations/referrals for utilization of services.
• Interfaces with Medical Directors, Physician Advisors and/or Inter-Disciplinary Teams on care management treatment plans.
• Directs and supervises the work of any non-RN clinicians in coordinating services for the member.
