Job summary

Morrow County is seeking a Full Time Nurse Case Manager/Community Health Educator to serve schools in Heppner and Boardman, with occasional home visits and outreach for community education including Tobacco Prevention Education.

Requires Oregon RN license and driver's license; BSN preferred. Public health experience may substitute for degree. Salary is $4,540/month for full-time with excellent benefits.

Responsibilities

• Provide nurse case management and community health education.
• Conduct home visits and school-based outreach in Heppner/Boardman.
• Assist with immunization clinics (on/offsite) and education.
• Screen and follow up on communicable diseases; provide family planning education.
• Promote healthy communities and work autonomously with diverse families.

Morrow County has a Full Time Nursing vacancy within for a Nurse Case Manager/Community Health Educator. This position is primarily based within the Heppner and Boardman area schools, may involve home visitation on occasion and includes outreach for Community Education including Tobacco Prevention Education. Extensive orientation provided. This position works with culturally diverse families in communities with broad based socio-inequity. May also assist with immunization clinics (on/offsite), screen and follow up regarding communicable disease, and provide family planning services and education.

QUALIFICATIONS: Current Oregon RN license and driver’s license required. BSN preferred. Public health or related experience may be substituted for degree. Bilingual (Spanish) preference.

SALARY: $4,540/month for full-time. Excellent benefits offered, including retirement.
