Job summary

Masonicare ALSA Services is seeking an RN for Assisted Living on a per diem basis to oversee resident care in the community.

The RN provides assessments, develops and evaluates care plans, supervises staff, manages medications, coordinates with the care team, and educates residents and families on procedures and wellness. Per diem schedule offers flexible, as-needed hours.

Qualifications

• RN Degree required.
• Two years full-time nursing experience (one year in home health care or community health required) or a diploma/Associate's Degree with four years full-time experience over the past 10 years (one year in home health care or community health required).

Responsibilities

• Performs initial and ongoing nursing assessments to establish a goal-directed resident plan of services.
• Develops, implements and evaluates resident plan of services.
• Medication management and physician order follow-through.
• Transcribes, implements and evaluates physician orders promptly.
• Educates, supervises and counsels residents and families regarding AL procedures and care needs.
• Coordinates the plan of services and maintains continuity of care with other professionals.
• Documents accurate information in a timely manner.
• Conducts supervisory visits of care staff as required.
• Organizes educational programs, in-services and workshops to enhance skills.
• Assesses client condition at least every 120 days or sooner if needed.
• Refers to appropriate professionals or agencies when needed.
• Plans for clients who no longer require ALSA services.
• Nursing services including client teaching, wellness teaching, health promotion, disease prevention.
• Administers medications and delegates supervision of self-administered meds as specified.

The ALSA Registered Nurse (AN) is responsible for overseeing the resident care services that are provided in the community.

Essential Duties and Responsibilities:

1. Performs initial and ongoing nursing assessments utilizing the nursing process to establish a comprehensive goal directed resident plan of services.

1. Develops, implements and evaluates resident plan of services.

1. Medication management and physician order follow through.

1. Transcribes, implements and evaluates physician orders in a timely manner.

1. Provides education, supervision and counseling to resident and family regarding AL procedures and other care needs.

1. Provides coordination of the plan of services and maintains continuity of care with other professionals.

1. Documents accurate, pertinent and comprehensive information in a timely and concise manner.

1. Conducts supervisory visits of resident care assistants, companions and homemakers as required.

1. Arranges and participates in educational programs, in-services, meetings and workshops and utilizes current literature to enhance professional skills and knowledge.

1. Assessments, completed as often as necessary based on client's condition but not less frequently than every one hundred and twenty (120) days, and prompt action when a change in the client's condition would require a change in the client's service program.

1. Referral to appropriate professionals or agencies, whenever the client's condition necessitates, including the provision of current clinical information ensuring that if the client's condition is no longer chronic and stable, services of a licensed home health agency are engaged or other appropriate arrangement are made.

1. Planning for clients who shall no longer receive or require the services of the assisted living services agency.

1. Nursing services, which shall include, but not necessarily be limited to:

1. client teaching

1. wellness teaching

1. health promotion

1. disease prevention.

1. Responsible for medication administration and delegation of supervision of self-administered medications as specified in subdivision (4) of the subsection.

1. Perform other duties as assigned.

Minimum Qualifications:

Education: RN Degree required

Experience: A Baccalaureate Degree in Nursing with two years full time experience (one year in home health care or community health required) or a diploma or Associate's Degree in nursing with four years of full time experience over the past 10 years (one year of home health care or community health required).

Certificates, Licenses, Registrations: RN License in good standing with the state of CT.
