Job summary

Bristol Hospice Visalia is seeking a compassionate Registered Nurse (RN) with strong assessment skills to join our hospice team in Visalia, CA. You will perform comprehensive patient and family assessments and provide skilled nursing care in accordance with the nursing process.

Responsibilities include developing and revising care plans with patient and family input, communicating with physicians, and educating families on self-care.

Qualifications

• At least 1 year of acute care experience in an institutional setting.
• 1 year home care experience preferred.
• Current state nursing license in good standing.
• Flexible in work hours and travel across Bristol Hospice service area.

• Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness.
• Provide professional nursing care utilizing all elements of the nursing process.
• Develop and revise the plan of care with patient and family input.
• Communicate with the physician regarding the patient’s needs and report changes.
• Teach the patient and family self‑care techniques as appropriate.
• Work in concert with the interdisciplinary team.

On an An Average Day you Will:

(Includes, but not limited to)

• Complete an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs
• Provide a complete physical assessment and history of current and previous illness(es)
• Provide professional nursing care by utilizing all elements of the nursing process
• Assess and evaluate patient’s status by:
• Writing and initiating plan of care
• Regularly re-evaluating patient and family/caregiver needs
• Participating in revising the plan of care, as necessary
• Initiate the plan of care and make necessary revisions as patient status and needs change
• Use health assessment data to determine nursing diagnosis
• Develop a care plan that establishes goals, based on nursing diagnosis
• Include the patient and the family in the planning process
• Record pain/symptom management and changes/outcomes as appropriate
• Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders, as required
• Teach the patient and family/caregiver self‑care techniques, as appropriate
• Work in concert with the interdisciplinary group
• All other duties as assigned

Requirements:

• Must have at least one (1) year of acute care experience in an institutional setting
• One (1) year of experience in home care preferred
• Must have a current licensure in the state
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate nursing skills per competency checklist
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
• Ability to travel throughout the Bristol Hospice Service Area
