Job summary

AdventHealth seeks a Registered Nurse for home health in Temple Terrace, FL. You will evaluate home environments for safety, provide skilled nursing care, and coordinate care plans with physicians and the healthcare team.

The role requires an Associate's Degree in Nursing (Bachelor's preferred), RN license, and 1+ relevant clinical home health experience. Travel to assigned counties is required; pay range listed is $33.11–$61.60 per hour with benefits from day one.

Responsibilities

• Evaluate the home environment for safety, infection control and community resources needs.
• Review patient history, tests and labs; report abnormal results to physician and implement care plans.
• Provide skilled nursing care and prescribed treatments in home settings.
• Use health coaching to engage patients in care management.
• Maintain clinical records and care plan updates.
• Develop patient-specific care plans with patient, family, and physician.

Job Description:

Mileage reimbursement provided: $0.76 per mile

Schedule: Monday - Friday, 8am - 5pm; weekend on-call rotation

• Evaluates the home environment for safety, infection control, and community resource needs.
• Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician. Implements care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities.
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments in various home situations.
• Uses motivational interviewing and health coaching techniques to engage stakeholders in care management.
• Informs the physician, clinical manager, and healthcare team of changes in the patient’s condition and needs.
• Maintains updated clinical records, meeting documentation deadlines for certification, re-certification, and care plan updates.
• Provides comprehensive assessment, planning, implementation, and evaluation for a caseload of home patients.
• Optimizes daily schedules to support productivity, efficiency, and best practice visit utilization.
• Assesses physical, functional, psychosocial, cultural, cognitive status, and discharge planning needs of home care patients.
• Formulates patient-specific care plans with the patient, family, and physician, establishing individualized, realistic, measurable goals.
• Other duties as assigned.
